Cognitive Health Assessment Programme (Programme) is a dedicated programme under our Neurocognitive Centre and is designed and led by neurologists. The Programme employs advanced blood-based biomarker diagnostics, supported by artificial intelligence assisted brain imaging assessment tool, AccuBrain®, which is more accurate and effective in detecting early-stage Alzheimer’s Disease and other neurodegenerative cognitive disorders.

Thanks to the generous contribution from the Lee Heng Charity Foundation, individuals who have been initially assessed by our neurologists as suitable candidates, and been referred to our Neurocognitive Centre for follow-up to participate in the ‘Cognitive Health Assessment Programme', may apply for financial support under the '"Cognitive Health Assessment Programme" Subsidy Scheme' (Subsidy Scheme) as listed below:

For example, for application submitted during the registration period of the ‘New Year Health Kick-off Offer 2025’, the Prevailing Price is the offer price under such offer, that is, HK$11,200, and the self-pay fee amount is HK$4,480 after deducting the Subsidy.

Terms and Conditions for the
Cognitive Health Assessment Programme Subsidy Scheme

  1. (a) These terms and conditions are applicable to the applicants (each an 'Applicant') for the subsidy provided under the ‘“Cognitive Health Assessment Programme” Subsidy Scheme’ ('Subsidy Scheme'). Once the subsidy application is submitted, the Applicant is deemed to have read, understood, and accepted these terms and conditions, all the terms and conditions set out in the application form, and all other rules as stipulated by CUHK Medical Centre Limited ('CUHKMC') for the Subsidy Scheme, and agreed to be bound by all of the foregoing. 

    (b) For an Applicant who has been approved of the subsidy provided under the Subsidy Scheme ('Subsidy') participates in the 'Cognitive Health Assessment Programme' ('Programme'), if the Prevailing Price (as defined in clause 3 below) on which the Subsidy amount is calculated is an offer price, any term that imposes time limit on receiving services under the related offer (if any) as set out in the terms and conditions for such offer shall not apply to such Applicant.    
     
  2. To be eligible for the Subsidy, an Applicant must meet the following eligibility criteria and submit application in accordance with the prescribed procedure: 
    (i)    Applicant must be a holder of a valid Hong Kong Identity Card;
    (ii)   Applicant must have been initially assessed as having symptoms of dementia or neurodegenerative cognitive disorder by neurologist of CUHK Medical Centre, and is being referred by such neurologist to the Neurocognitive Centre of CUHK Medical Centre for follow up;
    (iii)   Applicant must not possess any medical insurance coverage or be the beneficiary of any medical insurance or coverage (excluding employee medical insurance coverage provided by employers); and
    (iv)   Applicant has to meet one of the financial requirements set out in clause 3 below.
     
  3. The levels of the Subsidy are categorised according to the financial requirements below met by the Applicant:

    ^ The Subsidy amount is calculated based on the prevailing listed price of the Programme or the offer price of the Programme under a prevailing offer (subject to fulfillment of the requirements under such offer) on the date of Subsidy application submission (‘Prevailing Price’), such date being the date of receipt of application by the Neurocognitive Centre if submitting in person or the date of the post office stamp if submitting by post.

    These self-pay fee amounts are calculated based on the offer price under the prevailing ‘New Year Health Kick-off Offer 2025' of HK$11,200 that is valid from 15 January 2025 to 30 April 2025 (both days inclusive) (which is the registration period of such offer) for reference only. 
  4. Application procedure: Applicant must submit a duly completed and signed prescribed application form and provide documentation for supporting the financial requirements and all other information and documentation as requested by CUHKMC in accordance with the requirements of CUHKMC. CUHKMC will assess and verify the eligibility for the Subsidy of the Applicant based on the information and documentation provided by the Applicant. The decision of CUHKMC made on the applications shall be final and conclusive.
  5. The Subsidy can only be used to pay for/deduct from the fee (in full or in part) of the Programme. Any medical consultation, treatment, procedure, investigation, medication, or service provided beyond the scope of the Programme shall be at the Applicant’s own costs.
  6. Due to the nature of the Subsidy Scheme, once the amount of donation designated for the Subsidy Scheme has been depleted, CUHKMC will stop providing any Subsidy and the Subsidy Scheme will terminate without prior notice; that is, even if an application is submitted, the Applicant may not receive the Subsidy and has to pay the full fee amount for the relevant service at the Prevailing Price on the date of receiving the service.
  7. The Subsidy is not transferable and cannot be exchanged for cash or other products or services. Any fee self-paid by an Applicant under the Subsidy Scheme is not refundable and cannot be exchanged for other products or services.
  8. CUHKMC reserves the right to amend these terms and conditions at any time without prior notice.
  9.  In case of any dispute, the decision of CUHKMC shall be final and conclusive.
  10. These terms and conditions shall be governed by, and construed in accordance with, the laws of the Hong Kong Special Administrative Region. No person other than the Applicants and CUHKMC will have any right under the Contracts (Rights of Third Parties) Ordinance (Cap. 623) to enforce any of these terms and conditions.
  11. In the event of inconsistency between the English and the Chinese versions of these terms and conditions, the English version shall prevail.
