Terms and Conditions
- Subject to these Terms and Conditions, the discount offer (the “Group Offer”) on Plan A Integrated Assessment Plan and Plan B Injury Risk Assessment Programme of the Sports Well Programme (the “Programme”) is only available to participants joining the Programme through their respective sports event organizers, running clubs, or groups each with more than 10 individual members (“Participants”).
- The promotion period is from now to 30 April 2025, both dates inclusive.
- The Group Offer is only applicable to CUHK Medical Centre, and is on a first-come, first-served basis subject to the service availability of CUHK Medical Centre.
- Services under the Group Offer shall be managed by Registered Medical Practitioners and, if applicable, Physiotherapists, as designated by CUHK Medical Centre.
- To enjoy the Group Offer, a Participant must make an advance booking via his/her relevant sports event organizer, running club or group with more than 10 individuals, and complete the services under the Programme on or before 30 April 2025. Booking can be made by calling or sending WhatsApp Message to the Business Development Team of CUHK Medical Centre at 5213 2232 during service hours (Monday to Friday: from 9:00 a.m. to 5:00 p.m. and Saturday: from 9:00 a.m. to 1:00 p.m. (excluding public holidays)).
- All items in each of the plans under the Programmes are fixed. In case there is any item in any plan being declined by the Participant or not conducted due to any reason, there will be no refund or exchange for such item.
- The Group Offer cannot be redeemed or exchanged for cash or other products and/or services and the Group Offer is not transferrable or refundable.
- The Group Offer cannot be used in conjunction with any other offers, discounts or promotions.
- CUHK Medical Centre Limited reserves the right to amend any of these Terms and Conditions at any time without prior notice.
- In case of any dispute, CUHK Medical Centre Limited’s decision shall be final and conclusive.
- These Terms and Conditions shall be governed by, and construed in accordance with the laws of the Hong Kong Special Administrative Region.
- In the event of inconsistency between the English and the Chinese versions of these Terms and Conditions, the English version shall prevail.
